An internal investigation has been launched, following the death of a woman at Newcastle’s John Hunter Hospital.
The female patient reportedly was missing for almost eight hours before she was found dead in a toilet metres from her bed on May the 12th.
The incident has sparked questions about why the alarm wasn’t raised sooner.
Hunter New England Health’s Chief Executive, Michael DiRienzo, says an investigation is currently underway.
A number of staff have also been disciplined, pending the findings.
