A 40-year-old woman has been charged over a fatal crash at Old Bar in January.
The woman was taken to Waratah Police Station on Friday and charged with multiple offences including manslaughter and dangerous driving occasioning death.
She was granted strict conditional bail to appear at Taree Local Court in March.
