The Pacific Highway, north of Taree, was blocked on Monday night by a serious accident at Johns River.
The crash happened on the Stewarts River Bridge, and involved a truck and a number of cars.
One person is dead and another trapped.
Two rescue helicopters were sent to the scene.
Traffic was diverted via the Oxley and new England Highways.
