SYSYEM FAULT MAY HAVE CAUSED DEATH OF A MAN IN A SINGLETON MINE

An inquest has found a problem with a coal mine sensor system might have caused the death of a man in a Singleton mine in 2009.

59-year-old David Oldknow was crushed to death on February 18, 2009 when 10 tonnes of waste rock crashed onto his truck.

The inquest heard it is possible debris blocked the sensor beam, causing waste to fall prematurely or that there were issues with the programmable logic control system.

The inquest continues.
